The Tour Manager will then send the forms together with a summary spreadsheet to Worldsport. Examples are given, please complete the relevant spreadsheet using the formats given as an example.

If you do not have all the information to complete the form, please send what you have as soon as possible - additions can be easily made later.

Medical costs are not included in travel insurance for Australian or New Zealand residents as they are covered for emergency medical expenses under Medicare (treatment MUST take place at a public hospital) and a reciprocal arrangement between these countries. (It is against the law for travel insurance to cover medical costs under these conditions.)
